Benefits at a Glance
Short term disability insurance will help ease the financial stress of a covered illness or injury.
Each full-time employee may select a benefit coverage in $50 increments not to exceed 60% of current salary with a maximum of $1,250/weekly benefit.

Plan Design
- Benefits begin on the 15th day of disability due to an accident or sickness. Benefits are payable as long as you are totally disabled, up to 11 weeks, or 90 days.
- Maternity Benefits are paid the same as for an illness.
- Benefits are not paid for occupational illness or accident.
- Disability benefits begin after your sick-leave is exhausted.
Benefits Offset
- Amounts of weekly benefits will be reduced by any Social Security or other retirement income being received by the claimant.
- No benefit will be paid if you are injured on the job or collecting Workers Compensation benefits.
Certificate of Coverage
Pre-existing Limitation
Benefits will not be paid as a result of a “pre-existing condition” until the insured has gone twelve (12) months treatment free or has been continuously covered for twelve (12) months. A pre-existing condition is an illness or injury for which the insured has received diagnosis, treatment or advice with in a three (3) month period preceding the effective date of enrollment or an increase in coverage.
