The plan pays a lump sum payment of the maximum Critical Illness benefit amount upon diagnosis of a covered illness.

Rates are based on the age and amount of coverage.
Covered Dependents receive 50% of the Employees Benefit.

A partial (potentially multiple) benefit of 25% of the maximum critical illness benefit amount will be paid upon diagnosis of Coronary Artery Bypass. *
The Plan Also Includes Additional Enhancement Riders:
- Reoccurance of the initial illness
- Cardiopulmonary Enhancement Rider: 25% benefit for: Cardaic Arrest, Pulmonary Embolism, Pulmonary Fibrosis
- Lifestyle Enhancement Rider: $25 for completing cessation program, online health assessment, health challenge.
- $50 Wellness Rider

Older Voya Policy, No Longer Offered
Benefits will be reduced by 50% on the certificate anniversary following the insured’s 70th birthday. The premiums will not decrease.
