Benefits At a Glance
The specified critical illness policy pays a lump sum benefit, based on the date of diagnosis, up to 100% of the policy’s face amount if you are diagnosed with one of the following critical illnesses. You may choose a face amount from $5,000 to $50,000 in $1,000 increments.

Benefit terminates when 100% of face amount has been paid.
Plan Benefits
If the Primary Insured or Spouse, as shown in the Policy Schedule, receives a benefit for Carcinoma in Situ and is later diagnosed with Cancer or with a different covered Specified Critical Illness as shown in the Policy Schedule, benefit will equal be less the amount received for Carcinoma in Situ or Coronary Artery Bypass Surgery.
Unmarried children are covered to age 25, if they are residing with and dependent upon employee for at least 50% of this support.
Annual wellness benefit of $50 per year per covered person. There is a 30-day waiting period from effective date for benefit and wellness benefit.
