Dental Insurance
The Dental Insurance plan is changing to MetLife Effective 1/1/2025.
The current plan is insured by United Concordia.
All retirees have two options. You may select a traditional dental insurance “High Option” plan or, a more economical “Low Option” plan. With either option, employees may see any licensed dentist they choose. If you choose to see a dentist in the network, you will have lower out-of-pocket costs, thereby saving you money.
Rates
Monthly Premiums will be deducted from your pension checks.
Rates are current with UCCI and Effective January 1 with MetLife (2.8% reduction)

* There is a 12 month waiting period for Major services. The waiting period will be waived if coverage is taken when first offered and there has been no break in dental coverage. (i.e… coming off of Cobra, the Cobb County Dental Plan or other insured dental coverage).
** Deductible applies to Basic and Major services. Waived for Preventative services.
*** Maximum Allowable Charge – The Maximum Allowable charge for all services, including treatment for dental emergencies, to a Non-Preferred Dentist will be equal to or greater than Maximum Allowable charge to a Preferred Dentist/contracted Dentist.

Locate a Dentist
Through December 31, 2024
- Go to the United Concordia website at www.unitedconcordia.com
- Click on “Find a Dentist”. Choose “Elite Plus”
- Search by either your zip code, city, or your current dentist’s name
The resulting list will provide you with a current listing of all of United Concordia’s preferred dental providers that have agreed to save you money! This benefit is insured by United Concordia. Refer to your certificate of coverage or the master policy contract for exact provisions and specifications.
Beginning January 1, 2025
There are thousands of general dentists and specialists to choose from nationwide –so you are sure to find one that meets your needs. You can receive a list of these participating dentists online at www.metlife.com/mybenefits or call 1-800-438-6388 to have a list faxed or mailed to you.
You are always free to select the dentist of your choice. However, if you choose a non-participating dentist your out-of-pocket costs may be higher.
Retirees can register your account to search for providers, and print an ID card online through https://online.metlife.com/ (Search for Cobb County Retired Employees Association after January 1) – REGISTER for your account first.
Employees can use the MetLife Mobile App – Search MetLife Dental from the APP Store
Smile for Health® – Wellness through 12/31/2024
If you have been diagnosed with one of the following chronic medical conditions, you are eligible for improved dental benefits through Smile for Health®–Wellness.
Cerebral Vascular Disease (including Stroke) | Oral Cancer |

With Smile for Health®–Wellness, eligible members receive full coverage for necessary periodontal services, including surgical procedures, to remove the financial obstacles to total oral care.
How to Register
You and any eligible dependents can register to use Smile for Health®–Wellness any time after your insurance plan’s effective date. Here’s how:
- Visit https://UnitedConcordia.com/GetMDB from your desktop or mobile device
- Sign into My Dental Benefits (or Start a member account)
- Click the Wellness tab at the top menu
- Click the +Add a new condition and complete the fields as prompted
- Your condition status will show as ACTIVE to confirm your activation
Once registered, talk to your dentist about what (if any) additional services you may need to keep your mouth as healthy as possible.
