Eligibility
Employees can cover dependents to age 26. Spouse coverage is available. Surcharges may apply.
Maternity benefits apply to employees/spouse, not dependent children.

Early Retirees (under 65):

Anthem Drug List
Your plan uses the National Drug List. It includes hundreds of generic and brand-name prescription drugs.
You can get a sneak peek of your drug list to see if a drug you take is covered and select the National 4 Tier.
For more details, log in at anthem.com. If your drug isn’t on the list, you’ll see other options. Keep in mind, changes can be made to your drug list. So you may want to check it when you get a new prescription.
Drugs are grouped in tiers. Your share of the cost will depend on which tier your drug is on. The lower the tier, the lower your cost.
Your plan uses the Preferred Generics program. This means when there’s a generic option available and you choose to go with the brand-name drug instead, you’ll pay more. Check with your doctor to see if there’s a generic option that’s right for you — it’ll save you money!
Specialty Pharmacy
If you have a complex health condition that requires specialty drugs for your treatment — drugs that may need special handling or that you may get by injection or infusion — you’ll need to get them through IngenioRX, our specialty pharmacy. Kaiser Wellness Clinic Specific Information
Approximately 40 different types of generic medications are offered and will be made available to employees at the time of treatment.
Best of all, there is no cost to eligible employees or their spouses and dependents that are covered under a county health insurance plan for the medications dispensed at the clinic.

Getting Started
- So your needs can be efficiently accommodated, an appointment is needed to come to the clinic to receive medication there.
The number to call for an appointment is 770-528-1924. - Make the appointment before running out of any medication completely. There should be approximately one to two weeks of medicine left at the appointment time. This will allow time if there is a need to order medicine– don’t run out!
- Bring proof of your prescriptions the first appointment time. This can be a written prescription from another provider, a current bottle from a pharmacy, or paperwork from a pharmacy indicating the last time the medicine was filled there.

Policy and Procedures – Updated
If a missed appointment occurs due to work obligations, the fee will be waived if an employee provides a “No Show Verification” form from their supervisor. No Show Verification forms must be given to the clinic staff within five (5) days of a missed appointment.

- The Wellness Clinic can help counsel and prescribe medication for employees to quit smoking.
